Healthy Lifestyles Coventry (HLS) has been commissioned by Coventry County Council to provide lifestyle advice and support including weight management and smoking cessation. The service is operated by ICE Creates Ltd.
To provide these services, we need to process (which means use) your personal data. We may also deliver our services through GP Surgeries and pharmacies which would mean sharing your personal data with them.
Your personal data will only be shared with other services with your consent or for legal reasons. Your personal data may also be shared with regulatory and statutory bodies who assess council performance and financial spend, as well as where required by law, such as to prevent and detect crime or fraudulent activity.
Your data is used to assist us in delivering the most suitable lifestyle services to meet your specific needs; for our coaches to accurately monitor your progress throughout the programme; to help us to make sure that our services remain current and effective; to enable us to offer you a better service by sharing your health information with your GP.
We will also use your data to better understand the services we provide and to help build those services for the future. We may also use your data to identify if our services are fulfilling our legal obligations.
You are consenting to your personal data being processed to provide you with lifestyle advice and support, which will allow your coach to deliver these services to you. HLS will retain your personal data for 5 years, although there may be a legal reason for or piece of legislation telling us to keep it longer.
You consent to your personal data being shared with your GP and pharmacies, where we deliver our services through them.
You consent to your details being used for statistic and analytical purposes. These reports will be fed back into Coventry County Council and the Department of Health and Social Care to evaluate the service.
Your data subject rights include revoking consent, accessing your information, correcting inaccurate information, erasure and objection.
